VILLA LIFESTYLE INFORMATION FORM - If your real estate agent is requesting this form, there is a $250.00 completion fee which is invoiced to the SELLER (or Agent if they request) and can be paid when received, or at closing. If the property does not close, this invoice is due upon receipt. It must be requested by the entity requiring the document (Seller or Agent).
